Job Description

General Purpose of Job:

Provides nutritional services to patients including assessing/re-assessing individual nutritional needs, developing nutritional care plans, counseling, and facilitating diet and diabetes instruction for individuals and groups.

Essential Duties:

This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.



  • Meets with patients to assess individual nutritional needs and develop a nutritional care plan based upon physician instructions and patient food preferences.
  • Plans, develops and facilitates diet and diabetes instruction for individuals and groups; develops related instructional materials.
  • Works in conjunction with a multi-disciplinary care coordination team to follow CCM and TCM patients, providing nutrition assessments by phone or in person and developing a nutritional care plan based upon patient's needs.
  • Facilitates individual and group instruction for Cardiac Rehab patients including nutrition workshops and cooking classes.
  • Community outreach programs/presentations to including workshops, seminars and community garden at AL!VE.
  • Serves as a Preceptor for dietetic interns and medical students.
  • Provide coverage for clinical inpatient dietitian as needed: communicates regularly with physicians and nursing staff to monitor patient's nutritional status, response to treatment, etc.; recommends dietary changes as needed. Documents patient assessments, food preferences, nutritional care plan, and food interactions in a timely manner and in accordance with Hospital policies.
  • Attends staff meetings and in-service programs as required.
  • Supports and serves as a role model for our mission, vision, values, and customer service initiatives. Adheres to the organization's policies & procedures, and compliance guidelines.
  • Performs other duties as assigned.




Job Requirements



General Requirements * Valid Registered Dietitian licensure * One of the following requirements must be met within 3 months of hire: * Certified Diabetes Educator (CDE) * 15 hours of approved Diabetes continuing education within one year prior to assuming the role or within three months of assuming the role
Work Experience * Previous experience with hospital clients * Experience in diabetes education - preferred
Education * Bachelor's degree in Dietetics
Specialized Knowledge and Skills * Knowledge of nutritional values of food and the therapeutic values of planned menus * Knowledge of regulatory standards and hospital policies related to assigned function including HIPAA confidentiality, State of Michigan, ADA, and TJC mandates. * Written and computer skills necessary to prepare required documentation * Interpersonal skills necessary to effectively communicate with patients, families, physicians, and health care professionals in the exchange of information. The incumbent is also required to effectively teach adult learners with varying learning abilities and understanding of the English language * Analytical ability to assess patient's nutritional needs and develop a nutritional care plan based upon physician instructions and patient food preferences * Mental ability to adapt and respond to multiple priorities and demands and meet the instructional needs of patients and departmental staff * Physical ability to transport instructional materials/equipment weighing up to 50 pounds * Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job
